Have you ever wanted to know… Are You Getting the Most Out of Your Medicaid Managed Care Plan?

Representatives from Chisel Collective will provide Health-Related Social Needs (HRSN) screenings for individuals enrolled in a Medicaid Managed Care (HMO) plan. As part of New York’s Social Care Network, Chisel Collective helps connect eligible community members with enhanced services that address social needs affecting their health and well-being.

Depending on eligibility, participants may be connected with assistance for food, housing (including rental and security deposit assistance), utilities, transportation, and other essential needs. The representatives will also be available to answer questions about the program and the resources that may be available to Medicaid recipients.
